*Residents/Fellows must include letter of verification from program director with registration in order to qualify for the Resident/Fellow rate for meeting and dinner. If no letter is included, you will be charged the member rate. **Includes fees for both the Architectural Boat Tour on Friday, July 13 & Closing Dinner at the Notebaert Children’s Museum on Saturday, July 14. IMPORTANT: Please indicate the event(s) in which you will be attending by checking the appropriate circle below as well as the number of people attending. Please note that the Boat Tour or Notebaert Museum Dinner are not offered as separate admission events. A Social Event Ticket must be purchased whether you plan on attending one or both events. Online registration HOTEL ACCOMMODATIONS The Raffaello Hotel (overflow accommodations - 1 block from the Westin) 201 East Delaware Place Chicago, IL 60611 (312) 943-5000 www.chicagoraffaello.com **IMPORTANT** Due to heavy demand, the Westin Chicago Michigan Avenue, site of SPD 2007, is no longer able to provide rooms at the advertised group rate of $179 per night. There are still a handful of rooms available at the Westin during the SPD Conference, however, at rates that exceed $275 - $300. As a result, SPD has made arrangements for overflow accommodations at The Raffaello Hotel, located at 201 E. Delaware Place, right behind the John Hancock Building. The Raffaello is only about a block from the Westin. The Raffaello recently underwent a $25 million dollar renovation and offers rooms with one king bed or two double queen beds, 32" LCD TV, Gilchrest & Soames amenities, Wi-FI Access, rainforest showerheads and state-of-the-art-fitness and wellness facilities. Starting Wednesday, June 6, rooms will be available at the rate of $199 per night (single) and $219 per night (double) and may be reserved by calling the Raffaello at (312) 943-5000, then enter ext. 0 for the front desk and let them know that you are booking for the Society for Pediatric Dermatology (Group Code SPD). AIR TRAVEL INFORMATION & TRANSPORTATION O’Hare International Airport (ORD) The Westin Michigan Avenue is located approximately 40 minutes from the airport (18 miles) and car rental and taxi services are available at the airport. One-way taxi service from the airport to the hotel will cost approximately $40-45. Midway International Airport (MDW) The Westin Michigan Avenue is located approximately 30 minutes from the airport (15 miles) and car rental and taxi services are available at the airport. One-way taxi service from the airport to the hotel will cost approximately $30-$35. Attendees can also take the Chicago Transit Authority (CTA) to and from both airports; a one-way train fare is $2. From Midway, take the Orange Line to the Clark and Grant Street Station and hail a cab ($7-$10) to the Westin. From O’Hare, take the Blue Line to Clark and Grant Street Station and hail a cab ($7-$10) to the Westin. CME CREDIT This activity has been planned and implemented in accordance with the Essential Areas and policies of the Accreditation Council for Continuing Medical Education through the joint sponsorship of Blackwell Futura Media Services and The Society for Pediatric Dermatology. Blackwell Futura Media Services is accredited by the ACCME to provide continuing medical education for physicians. Blackwell Futura Media Services designates this educational activity for a maximum of 13.25 AMA PRA Category 1 Credits TM. Physicians should only claim credit commensurate with the extent of their participation in the activity. This continuing medical education activity has been reviewed by the American Academy of Pediatrics and is acceptable for up to 13.25 AAP credits. These credits can be applied toward the AAP CME/CPD Award available to Fellows and Candidate Fellows of the American Academy of Pediatrics. SPD’s Annual Meeting is also recognized by the American Academy of Dermatology for 13 hours of AAD Category 1 CME credit and may be used toward the American Academy of Dermatology’s Continuing Medical Education Award. SPECIAL EVENTS & ACTIVITIES All social activities and special events are open to physicians who are registered for the meeting and their spouse/guest and family members. Advance registration is required for all special events where noted, however, a minimum number of tickets may be available on site. Thursday, July 12 Fellow and Young Leaders in Peds Derm Reception 6:00 pm 7:00 pm If you are a fellow or out of training for five years or less, then this event is for you! Come network and share experiences with other young dermatologists. Sponsored by the newly-formed SPD Young Leaders group, the reception will feature a short presentation from Adelaide Hebert, MD, SPD President, who will share useful tips in her “Things I Wish I Would Have Known When I Was a Young Dermatologist” talk. (No cost to attend, however, pre-registration is required.) Welcome Reception 7:00 pm 10:00 pm Greet your friends and colleagues at the Welcome Reception while enjoying cocktails, hors d’ oeuvres and entertainment. Badges will be required for entry. Friday, July 13 Architectural Boat Tour 2:30 pm 5:00 pm Marvel at Chicago’s soaring towers while enjoying a 90-minute, narrated river cruise. This tour spotlights more than 50 architecturally significant sites and will provide a new perspective on the city. Come aboard either the Chicago’s First Lady or Chicago’s Little Lady, where both open-air and climate-controlled indoor seating will make your journey comfortable. Snacks and beverages will be provided as well. Buses depart the Westin at 2:30 pm with boat departure at 3:15 pm and return at 4:45 pm. (Cost: Included in Social Event ticket - $100 per adult, $50 per child; pre-registration is required.) Saturday, July 14 A Night at the Notebaert Nature Museum: Reception & Dinner 6:00 pm 9:30 pm Connect with science and nature through innovative exhibits exploring the bio-diversity of the world around us. The Notebaert Nature Museum boasts nearly 250,000 specimens, including plants, animals, fossils, and minerals. Attendees will enjoy a plentiful selection of hot and cold hors d’oeurves and a variety of beverages while viewing several of the exhibits. In addition to providing thousands of children with an accessible scientific education, this cornerstone center of the Chicago Academy of Sciences supports environmental research and conservation programs. (Cost: Included in Social Event ticket - $100 per adult, $50 per child, pre-registration is required; suggested age for this event 8-18.) PERSONS WITH DISABILITIES SPD provides services for persons with disabilities. If special arrangements are required for an individual to attend this conference, please notify the management office at (317) 202-0224. CANCELLATION/REFUND POLICY Registration fees, less a $100 administration fee, will be refunded following the meeting. In order to qualify for a refund of fees, cancellation notice must be received in writing no later than June 10, 2007. There will be no refund of fees for cancellation received after that date. KIDS CAMP: Friday, July 13 Sunday, July 15 SPD will offer a complimentary Kids Camp each day of the meeting from 8:00 am 1:30 pm. Children 3 years old and above are welcome to participate in fun activities such as Sports Challenges, Arts & Crafts and special themed programs. Space is limited so register early!
