*Residents/Fellows must include letter of verification from program director with registration in order to qualify for the Resident/Fellow rate for meeting and dinner. If no letter is included, you will be charged the member rate.

IMPORTANT: Please indicate the event(s) in which you will be attending by checking the appropriate box on the registration form as well as the number of people attending.

A special room rate of $139 per night single/double is available for all meeting participants at at the Snowbird Resort. Please indicate you are with the “Society for Pediatric Dermatology” to receive the special room rate. This rate is offered on a space-available basis only through June 18, 2008. Reservation requests made after June 18 will be confirmed on a space-available basis at prevailing hotel rates. Please note there are a limited number of double rooms available, so make your reservations early.

To make room reservations online, please click here and follow these steps:

1. Choose Buy online from the menu bar then click on lodging.
2. From there you will put in your dates and number of people per unit.
3. Enter the SPD group number (2AK5Q7) in the appropriate field. 
4. Click on the submit button and the $139 rate will be displayed.

Please note that full payment is due on all reservations booked on line, however the hotel's cancellation policy is that reservations will be fully refundable up to 48 hrs. prior to the arrival date for anyone who needs to cancel their reservation. 

IMPORTANT: Please remember to book and prepay the airport shuttle service when making your lodging reservations. See Airport Shuttle Service below for more information.

NEED A ROOMMATE?
Contact the SPD office at (317) 202-0224 or via email at spd@hp-assoc.com no later than June 1, 2008 if you would like to share a room with another attendee (and split the cost of the room). Please indicate whether you are male or female and your anticipated arrival and departure dates.

AIR TRAVEL INFORMATION & TRANSPORTATION
Salt Lake City International Airport is served by 16 airlines offering over 800 scheduled flights per day. The airport is 29 miles (40 minutes) from Snowbird Ski & Summer Resort.

Driving Directions (from the Airport): Take I-80 East to I-215 South. Take Exit 6 (6200 South) and go east on 6200, toward the mountains. This road will lead you straight to UT-210 and up Little Cottonwood Canyon to Snowbird. If you are driving to the resort, please note that hotel parking is free.

Airport Shuttle Service: Airport shuttle service is provided by Canyon Transportation. The cost is $60 roundtrip per adult ($34 one way) and $40 roundtrip per child 12 and under. Transportation must be booked and prepaid along with your lodging reservations. If shuttle service is not secured prior to arrival, the rate may vary depending on the transportation company. Prices are subject to change. Shuttles depart the airport every 20-30 minutes between 8:00am and 11:00pm. After hour shuttle service can be arranged at an additional cost. Reservations are required for the best service available. Please note that taxi service is available from the airport but ranges from $70 - $80 one way.

Arrivals: After claiming your luggage, proceed to the Ground Transportation desk, located adjacent to the baggage claim. If you have pre-paid your transportation, Canyon Transportation will have your name on file. It is recommended you keep your Snowbird confirmation with you as proof of prepayment. If you have not prepaid for your transportation, you can at this time. It is recommended that you purchase a round trip ticket. Tickets can be purchased with cash or major credit card.

Departures: Pick up time for the return trip should be set up when round trip ticket is purchased. Return reservations need to be confirmed at least 24 hours in advance. If a return ticket is not purchased in advance, a reservation must be placed 24 hours prior to departure with the concierge or by calling Snowbird Central Reservations at 1-800-453-3000.

CME CREDIT
This activity has been planned and implemented in accordance with the Essential Areas and policies of the Accreditation Council for Continuing Medical Education through the joint sponsorship of Blackwell Futura Media Services and The Society for Pediatric Dermatology. Blackwell Futura Media Services is accredited by the ACCME to provide continuing medical education for physicians.

Blackwell Futura Media Services designates this educational activity for a maximum of 13.25 AMA PRA Category 1 Credits TM. Physicians should only claim credit commensurate with the extent of their participation in the activity.

SPD’s Annual Meeting is also recognized by the American Academy of Dermatology for 13 hours of AAD Category 1 CME credit and may be used toward the American Academy of Dermatology’s Continuing Medical Education Award.

This continuing medical education activity has been reviewed by the American Academy of Pediatrics and is acceptable for up to 13.25 AAP credits. These credits can be applied toward the AAP CME/CPD Award available to Fellows and Candidate Members of the American Academy of Pediatrics.

SPECIAL EVENTS & ACTIVITIES
All social activities and special events are open to physicians who are registered for the meeting and their spouse/guest and family members. Advance registration is required for all special events where noted, however, a minimum number of tickets may be available on site.

Fellows & Young Leaders in Peds Derm Reception
6:00 pm – 7:00 pm
If you are a fellow or out of training for five years or less, then this event is for you! Come network and share experiences with other young dermatologists.
(No cost to attend, however, pre-registration is required.)

Welcome Reception
7:00 pm – 10:00 pm
Greet your friends and colleagues at the Welcome Reception while enjoying cocktails, hors d’ oeuvres and entertainment. Badges will be required for entry.

Guided Horseback Tour
2:30 pm or 4:00 pm (two time slots)

Enjoy a professionally-guided horseback tours of Mineral Basin, located on the other side of the mountain from the resort. The horses are both gentle and trail broken and the wranglers have extensive guiding knowledge. Pre-registration is required. Limited to the first 32 who register (16 per time slot) — please indicate your preferred tour time on the registration form.

Guided ATV Tour
2:30 pm or 4:00 pm (two time slots)

Enjoy a professionally-guided ATV tour of Mineral Basin and American Fork Canyon. The tour starts with a picturesque tram ride to Hidden Peak (11,000 ft). where you will be escorted by guides to the bottom of Mineral Basin. The ATVs are safe and easy to operate for first time or experienced riders. Pre-registration is required. Limited to the first 32 who register (16 per time slot) — please indicate your preferred tour time on the registration form.

Whitewater River Rafting
2:45 pm – 7:00 pm
Bus departs Snowbird at 2:45 pm (45 min. bus ride)
Riverrafting 4:00 pm – 6:00 pm
Bus arrives back at Snowbird – 7:00 pm

Take a trip down the Provo River through one of Utah’s “postcard” canyons, flanked by 11,880 ft. Mount Timpanogos. Expect to get splashed but not soaked! The trip includes state-of-the-art rafts, life vests, friendly guides and complimentary soft drinks. What to bring: camera, sandals or shoes or other appropriate footwear, swimwear or shorts, sunglasses and sunscreen. For pictures and video of the tour, please visit: www.highcountryrafting.com. Pre-registration is required. Limited to the first 100 who register.

Guided Hike
3:00 pm – 5:00 pm

Start with a tram ride to Hidden Peak (11,000 ft) and then hike down the mountain with an experienced guide past old mines, colorful wildflowers and beautiful alpine scenery. Guides are well-versed in Little Cottonwood Canyon folklore, geology and mining history. Pre-registration is required. Limited to the first 100 who register.

Shuttle Service to downtown Salt Lake City
5:00 pm – 10:00 pm

The SPD will provide free roundtrip shuttle service from Snowbird to downtown Salt Lake City on Thursday, July 10 only (leaving every 30 minutes between 5:00 pm – 10:00 pm) so that attendees can explore the downtown area, shops and restaurants.

SPD provides services for persons with disabilities. If special arrangements are required for an individual to attend this conference, please notify the management office at (317) 202-0224.

CANCELLATION/REFUND POLICY
Registration fees, less a $100 administration fee, will be refunded following the meeting. In order to qualify for a refund of fees, cancellation notice must be received in writing no later than June 10, 2007. There will be no refund of fees for cancellation received after that date.

KIDS CAMP
Thursday, July 10 – Saturday, July 12
Kids Camp sponsored by Stiefel Laboratories

SPD will offer a complimentary Kids Camp each day of the meeting.  Children 4 years old and above are welcome to participate in fun activities in beautiful Little Cottonwood Canyon!  Participants will explore the environment through nature hikes, science experiments and other mountain adventures. Space is limited so register early!

REGISTRATION
Parents will need to register for the Kids Camp BOTH with SPD and with the Camp Snowbird at the the hotel. To register with SPD, please indicate your child’s name and age in the appropriate space on the SPD registration form.  To register with Camp Snowbird, please download a registration form and fax it to (801) 933-2199 to the attention of Rebecca Turville, Director of the Snowbird Summer Camp.  You can also register with Rebecca via phone at (801) 933-2256 or email at rturville@snowbird.com.  Parents will be required to submit immunization records when registering.  Space is limited at the camp and all children must be registered direct with Camp Snowbird and on the SPD event registration form.

DROP OFF AND PICK UP TIMES
Kids Camp will begin at 7:30 am and parents can choose a pickup time of 2:00 pm or 4:30 pm on Thursday, July 10 and Friday, July 11.  Pickup time will be 1:00 pm or 2:00 pm on Saturday, July 12.  In order for your child to enjoy the full day of activities, the later pickup time is recommended, but parents will select one of the pickup times when they register for the Kids Camp.  When making your pickup time choice, please remember that on Thursday, July 10 SPD’s educational sessions conclude at 1:45 pm and the optional recreation activities, if you registered for them, begin at 2:30 pm. SPD’s educational sessions conclude at 1:55 pm on Friday, July 11 and at 1:00 pm on Saturday, July 12.

ADDITIONAL DETAILS
All children must be potty trained and are required to bring or wear the following items to Kids Camp.

-Sneakers (socks are recommended)
-Backpack
-Waterbottle and sunscreen
-Swimsuit (Water is part of every day at Snowbird Summer Camp. If your child can’t swim please indicate that on the childcare registration form and provide flotation device as necessary.)
-A jacket or sweatshirt due to the typical temperature change that occur at higher altitudes.
-Please remember that no electronic devices are allowed at camp. This includes MP3 players and hand-held games.

SPOUSES, CHILDREN & GUESTS
Please note that all spouses, children and guests must be officially registered to attend the meeting to participate in any of the meal functions (including breakfast) or social activities. Badges will be required to be worn during these activities to gain admittance.
